Prof. Zdzislaw Ploski sent in an extensive list produced by painstaking effort.
Aside from a few obvious errors, hardly anyone sees the same problems. If there's any lesson to be learned from this, more eyeballs are better when it comes to reviewing.

- Severity 2 bugs: Misspelling, misconstruction, or inconsistent usage (last update 18 December 2002).
- Severity 3 bugs: Minor typo or mistake not likely to cause confusion or misunderstanding (last update 18 December 2002).

Many of these corrections appear in the third printing (the printing number is on the bottom of page iv.) Corrections for bugs reported after May 2001 have not yet been published, but will be as soon as possible.
